With a planned gift to the National Headache Foundation, you can combine your desire to give to charity with your overall financial, tax and estate planning goals. Your planned gift gives you a special connection with the NHF. You will help those suffering from recurring headaches and migraines—for now and for years to come.

The following general forms are suggested:

Gifts by Will

Specific Bequest

“I give to the National Headache Foundation, whose national office is presently located at 820 N. Orleans, Suite 411, Chicago, IL 60610, [the sum of __ ($__) or describe property] to be used for general purposes (or you can specify for what purpose you would like your donation used).”

– or –

Residual Bequest

“I give to the National Headache Foundation, whose national office is presently located at 820 N. Orleans, Suite 411, Chicago, IL 60610, [all or percent (__%)] of the rest, residue and remainder of [my or the trust] estate, to be used for general purposes (or you can specify for what purpose you would like your donation used).”

The National Headache Foundation’s Tax ID number is 23-7073022.
